Please use this form to report an incident to the Club. If you have witnessed a breach of the code of conduct or a safety concern, this is the form to use. This form will go straight to the Club and you will be contacted by the Club within three business days.

A reminder that if you have an issue during a match and require assistance, you should report this to the anti social text number and get support from venue security.

DO

✅Be specific

Use clear, concise language and be as specific as possible

✅Provide objective and factual detail

Focus on the facts and what was witnessed

✅Include key details

Report the date, time, and location of the incident. Identify the individuals involved

✅Provide evidence if available

If you are able to, supply evidence such as a photo, video, or screenshot

✅Report promptly

Complete the report as soon as possible, while details are still fresh

DON'T

❌Generalise

General comments without details or facts cannot be investigated

❌Include second hand reports or comments

If you didn't witness it, it is not your incident to report

❌Provide personal opinions or assumptions

Leave it to the Club to investigate and come to a conclusion
